| Case Management (also referred to as service coordination) is the term used to describe the support provided to adults who live with their families or independently in the community. The agencies providing case management receive Government funding to provide this services. No fees are charged for this service.
How to Apply:
Case Management for Individuals Living with Family (Family Support)
If you already have a connection with an agency in the adult sector (referred to as your lead agency), talk to your contact person at that agency about the need for case management/family support. They can make the referral on your behalf.
First time callers to the adult sector can contact any of the agencies in the developmental sector to receive an ‘interim support’ visit (see integrated response) to discuss service and supports available in the adult sector and to be connected with a lead agency. The lead agency will make a referral to the family support wait list.

Case Management for Individuals Living Independently (Adult Protective Service Worker-APSW)
For individuals living independently in the community or wanting to do so in the near future, referrals can be made directly to the two Assessment Coordinators for the Adult Protective Service Worker (APSW) program by calling 416-963-8656.
Referrals for women should be directed to ext 231.
Referrals for men should be directed to ext 232.
